• Your name and a way to reach you
• The work you own and the catalog URL of the listing
• A statement of good faith and that the information is accurate
• A physical or electronic signature of the rights holder or authorized agent

Use the published support channel when available, and include “Copyright notice” in the first line. Repeat infringing listings may be unpublished.
